How to get your medical records

Medical records hold information about you. They are also sometimes called health records.

You'll have separate records for any NHS service you go to including your GP surgery, hospital, dentist or opticians.

How to get your record depends on which record it is.

If you wish to obtain a copy of your medical record or any information within your GP medical record, please contact reception who will explaint the procedure.

PLEASE NOTE YOUR GP PRACTICE ADMINISTRATION CANNOT HELP YOU WITH ONLINE HOSPITAL RECORD ACCESS MADE THROUGH 'MY CHART'.

Obtaining your GP Medical Records under GDPR

Under the GDPR a patient can request a copy of their GP medical records, in the first instance free of charge.  Subsequent requests may incur a fee.  If you wish to obtain a copy of your medical records please contact reception.  Please note copies of requested information can take up to 28 days to become available, sometimes longer if the record is extensive.  Records must be collected, with photo ID and signed for once they are processed.  We will keep these for 6 months only, then they will be destroyed and subsequent requests will incur a fee.

A patient can nominate another person to collect on their behalf.  Please advise reception of the details.
