If you’ve been off sick for 7 days or less

If you’re off work sick for 7 days or less, your employer should not ask for medical evidence that you’ve been ill. Instead, they can ask you to confirm that you’ve been ill. You can do this by filling in a form yourself when you return to work. We call this self-certification.

If you’ve been off sick for more than 7 days

To request a fit note, after using a self-certification form for the first 7 days of sickness absence:

  • You must make an appointment either by telephone or face to face with a GP to discuss obtaining a fit note (sickline/Med3).
  • If you have been in hospital as an in-patient, or have had a procedure at hospital or a private clinic and require more than 7 days off work, the hospital or clinic are responsible for providing you with a fit note to cover the time you need off work to recover. If once this initial fit note has run out and you are still not well enough to return to work, please make an appointment with a GP.
